Include platform definitions file common.h in firebird.h. Use OS-specific ThreadId instead artificial FB_THREAD_ID, avoiding unneeded casts in the code. This should fix BSD and MAC ports.

Fixed memory leak when unloading module, related with upgraded interfaces.
Fixed timer on posix to be high resolution.
