- meaningful ctor on Jrd::Lock, helping to avoid code dup
- avoid unneeded h-file dependencies, making boot build engine dependent

Make plugins use passed instance of master interface, not one got from fb_get_master_interface(). This helps in case when >1 fbclient libraries is loaded by process.
